Patients: To request a copy of your report, you must present the Records Request form and photo identification at:

Attorneys and other agencies: To request a report on behalf of a client, submit a written request on letterhead along with authorization to release medical information signed by your client.

NOTE; The duplication of medical records rates as authorized under Chapter 70.02 RCW and set forth in WAC 246-08-400 is one dollar and twelve cents (1.24) per page, up to thirty (30) pages, together with twenty-eight dollars ($28.00) for searching and handling of records.
